Generate only ANSI-92 SQL, rather than ANSI-89 SQL and then only ANSI-92 SQL for outer joins.

We are connecting to a new product Teiid to allow data virtualisation across data sources. This uses ANSI-92 as default and best practice, and hints for performance tuning across databases will not work if the SQL is written in ANSI-89. ANSI-89 is also an old standard that should no longer be used.
