Add back MySQL data source template

We noticed that MySQL data source template is removed from Spotfire Server, that's quite confusing.


We have lots of customers that utilize Information Link to retrieve data from MySQL, MySQL is commonly used and it's more than necessary to have a built-in function to connect to it.
